Osmotic diuresis from elevated blood glucose causes increased urination. When blood glucose is high, the kidneys struggle to reabsorb all the glucose, so glucose spills into the urine. Water follows the glucose into the urine, leading to a higher urine output. In a patient on total parenteral nutrition, the glucose load from the TPN can raise blood glucose quickly, so polyuria is a classic sign of hyperglycemia.

The other symptoms listed are less specific: hypotension can come from fluid shifts or other problems, while nausea and dizziness can occur for many reasons and aren’t reliable indicators of hyperglycemia on their own. If polyuria is observed, it’s important to check blood glucose and review the TPN regimen and insulin needs as ordered.
